When appropriate, we help connect you with participating qualified healthcare providers for medical evaluation and certification-related care.

04 — Navigate the Next Steps
If your healthcare provider determines that you meet applicable requirements, Elevate can help you understand what comes next and navigate available resources.

05 — Stay Supported
Your relationship with Elevate doesn’t have to end after your initial appointment. Ongoing navigation, wellness coaching, education, care coordination, and check-ins are available.
